Add 2 check boxes at the top or bottom of the form. "Send email only" and "Send Text only". Leaving them blank would still send to both.

Only do this when auto invite is enabled:
Add the two checkboxes on the external contact form and make it ignore SMS or Email that are setup in the invite campaign, if they are unchecked. Proceed with the campaign as the usual, just ignore unselected option.
Require that at least one of the options is selected.
